Specular Microscopy

Specular microscopy is a specialized imaging technique that examines the innermost layer of the cornea, called the endothelium. This single layer of cells is responsible for keeping the cornea clear by regulating its fluid content, and unlike most cells in the body, endothelial cells do not regenerate — so understanding their number and health is important for long-term corneal function.

How It Works

A specular microscope uses a specialized light and camera system to capture a magnified image of the endothelial cell layer. The device analyzes this image to estimate the endothelial cell density (the number of cells per square millimeter) and evaluate the size and shape uniformity of the cells. The process is non-contact and takes only a moment for each eye.

Why It's Used

Endothelial cell health is an important consideration before intraocular surgery, such as cataract surgery, since these procedures place some stress on the endothelium and a healthy cell reserve supports good corneal recovery afterward. Specular microscopy is also useful for monitoring corneal conditions that affect the endothelium, evaluating the cornea after contact lens wear or prior eye surgery, and establishing a baseline for future comparison.

What to Expect

This test is painless and non-invasive — the instrument does not touch your eye. You'll simply be asked to look into the device and hold still while it captures the image, and results are typically available right away for your provider to review as part of your evaluation.
